YOUTH PROGRAM PARTICIPANT INTAKE FORM

Chicago Mixed Martial Arts is committed to providing a safe training environment for ALL members. This form helps instructors and staff understand any medical conditions, injuries, or health concerns that may affect training or participation in classes.

All information provided will be treated confidentially and used only to support the member’s safety and well-being.

Martial arts can be an amazing space for children to build confidence, self-control, and connection—but it’s not one-size-fits-all. The best thing to do is to try out classes first. We provide a week of trial classes and encourage you to take advantage of this opportunity to try out as many classes as you can.

✅ Understand what you are signing up for (Brazilian Jiu-Jitsu is a ground-based martial art and will involve a lot of physical body contact).
✅ Visit and try out class(es).
✅ Talk to the coaches.
✅ See how your child responds.

The intake form will be used in conjunction with the trial classes in order to see if your child would be a good fit with our program and vice versa. If our coaching staff determines that your child isn’t a good fit for our program, please don’t take it as a personal rejection. It doesn’t mean your child can’t train or won’t thrive in martial arts. It is just that our program might not be the best fit for your child at this specific moment in time (the class structure, the style of training, or even the size and number of participants).

Sometimes it’s about safety—we may feel that we can’t give your child the support they need while also managing a busy class as well as be responsible for the physical safety and well-being of the other kids as well.

Sometimes it’s the environment—the pace and activities of the class might overwhelm your child and they might respond in a manner that is detrimental to the entire class.

Sometimes, we just don’t have enough time to explain all of this properly which can then make a “not a good fit” comment more offensive than the actual reasons as mentioned above. So please don’t take it personally. We are happy to have a proper discussion and provide you with some alternatives that might be better for your child!
